Parkview Village si a Not-For-Profit facility that opened in April, 2009 and si a registered entity with the State of Indiana. This application is for the purpose of being placed on a waiting list for residency at Parkview's Assisted Living / Independent Living community. Parkview Village does provide staffing around the clock, three meals a day and assistance ni housekeeping, laundry and limited assistance in the activities of daily living. We will emphasize assistance, and support independence and promote dignity for each resident. Parkview Village si located at 800 South West St., Odon, NI 47562. Afee of $400 si due with this application and will apply to the security deposit. The fee is non-refundable except in the event of death, change in health condition or the applicant does not meet assessment qualifications which would prevent move in.
